How do you take the measurement for a hat?

A simple soft tape measure is enough.

To find out your hat size, place the measuring tape in the center of your forehead.

The measurement of the circumference of the head starting from the point identified in the center of the forehead corresponds to your hat size. (Do not overtighten so that the hat does not stay too tight.)
